office to provide your dental care needs. We are committed to providing 

you with the highest quality oral health care in the most gentle, efficient 

manner possible.

On your first visit you can expect:

*An introduction to our office

*A thorough examination and review of your oral health, including necessary X-rays

*A careful evaluation of your dental status

*A treatment plan to meet your oral health needs

Your comfort, appearance, and long-range of dental health needs will be 

kept in mind at all times. When treatment is indicated, we will try to restore 

optimum dental health in as few, well-planned appointments as possible. 

Our primary goal is to help you keep your teeth for a lifetime. Enclosed you 

will find a health questionnaire. Please complete the questionnaire at your 

convenience and bring it with you to your first visit. If you have dental 

insurance, you will need to bring your insurance form completed and 

signed.

We recognize the value of your time. Except in emergency situations, you 

can expect us to be on time for you. We would appreciate the same 

courtesy from you. If for any reason you are unable to keep your 

appointment, we ask that you notify us 48 hours in advance by calling our 

office directly. We look forward to meeting you at your upcoming
